Clinical Attachment for PCF


The Clinical Attachment form was created to accompany the Proposal Clearance Form (PCF) for grant submissions involving clinical research. It provides information related to human subjects and hospital facilities or other entities which may be affiliated with the proposed study and aids in coordination of the review process.

The Clinical Attachment is available in Word format and should accompany the PCF whenever either PCF question 5 a, b or c is answered yes. Submit the PCF along with the clinical attachment to either the Office of Grants Management or the Office of Industry Contracts dependent upon the answers to PCF questions 5 a, b and c. Proposal submissions which are a clinical trial or involve one of the Louisville Medical Center hospitals (i.e. if question 5a or 5b on the PCF are answered yes) are submitted to Industry Contracts. Otherwise (i.e. question 5a or 5b are not answered yes on the PCF) the proposal is submitted to the Office of Grants Management.

For those proposals submitted to Industry Contracts, at the time the investigator learns that the proposal is likely to be awarded funding by the granting agency, then the research agreement/contract or notice of probable funding of the grant award should be submitted to the Clinical Research Services division along with a fully signed MIRA.
