The Occupational Health and Safety (OHS) Program supports the university’s teaching, research, and operational missions by proactively identifying workplace hazards, promoting safe work practices and partnering with departments to prevent injuries and occupational illness.
Through education, consultation, exposure assessment, and continuous improvement, OHS works collaboratively with the university community to foster a strong culture of safety and shared responsibility across all work environments.

Reporting Injury and Illness

Employees
Report all work-related injuries and illnesses to your supervisor immediately.

Supervisor
It is the supervisor's responsibility to submit the Worker's Comp Claim Form to Enterprise Risk.

- Any work-related fatality, amputation or incident requiring hospitalization must be reported immediately to DPS 502-852-6111.
Instructions for Specific Types of Injuries or Illnesses
- Call 911 or ULPD: 502-852-6111 and provide exact location and nature of emergency
- Includes such injuries as severe lacerations, severe burns, fractures, loss of
consciousness, and uncontrolled bleeding. - Perform first aid as trained or directed by 911 dispatcher
- Complete the risk management workers’ compensation form as soon as possible
- Supervisor: Call ahead to Campus Health Services 502-852-6446 and complete forms via risk management prior to seeking medical treatment.
- Go to the nearest Campus Health Services office during normal business hours: Campus Health Services Locations and Hours.
- Employees may seek treatment from any medical provider who accepts workers’ compensation insurance.
Flush mucous membranes with water and wash exposed skin with soap and water.

- Go the nearest Campus Health Services office during normal business hours: Campus Health Services Locations and Hours.
- All contaminated needle sticks and bloodborne pathogens exposures must be reported on Risk Management's Website.
- Inhalation: Take victim to fresh air.
- Skin or eye contact: Immediately flush exposed areas with water for 15 minutes utilizing a safety shower or eyewash station.
- Call 911 or ULPD: 502-852-6111
- Severe chemical exposures to the skin must be taken via EMS to an appropriate hospital.
- If possible, obtain the safety data sheet (SDS) for the chemical and send the SDS to the hospital with victim.
