The University of Louisville is dedicated to the protection and conservation of the environment. Established federal and state regulations require that the university ensure that hazardous, bio-hazardous, radioactive and universal wastes be properly managed “cradle-to-grave”. The Department of Environmental Health and Safety's (DEHS) Environmental Operations & Hazardous Materials (EOHM) Program is directly responsible for the management of these wastes for the University.

Please view this label tip sheet [PDF]. All chemical waste collection containers must have a DEHS 4L hazardous waste label attached [PDF].

Lab Close-Out Process
- Access the Lab Close-Out Guide [PDF] and procedures.
- Review the document well in advance of your target vacating date.
- For any questions about the checklist items, contact the appropriate DEHS staff member listed in the close-out guide. For general questions, please contact DEHS at 502-852-6670
- Submit the Lab Close-Out Notification form [sign-in required] to DEHS. ·
- Allow several weeks' notice for DEHS to arrange staff availability.
- Go through each item on the checklist. ·
- Mark items that do not apply as “not applicable” (n/a). ·
- Request waste pick-ups in advance to ensure all regulated waste is removed before the final inspection.
- If beneficial, contact DEHS at 502-852-6670 for a pre-lab closeout consult.
- The Principal Investigator (PI) or designee should provide a preferred date for the final inspection. ·
- DEHS will confirm the date and schedule the on-site visit.
- DEHS staff will conduct a walk-through inspection of the PI-assigned lab spaces and any shared lab spaces. ·
- If the inspection is satisfactory, DEHS will issue a lab close-out certification form.
- Copies of the signed form will be sent to the PI vacating the lab and the department head.
