All researchers must complete an Attestation and Disclosure Form (ADF) to fulfill the Conflict of Interest requirement. This form must be completed once annually.

To complete the Attestation and Disclosure Form, you will need to log in to iRIS, then fill out and successfully submit the form. Do not use Safari when completing any form in iRIS.

Please ensure you select “Involved in research” under your role(s) to complete the entire COI requirement for research. If you have any questions related to this requirement, please contact the COI Program. Additional information is available on the COI site.
